Who We Are:
The Geriatric Care Management program of JF&CS, now known as Your Elder Experts, was founded in 1999 and has grown into one of Greater Boston’s leading care management practices.
Our Expertise:
We are professionals, trained as social workers, counselors, and advocates, who can navigate senior services, housing options, medical information, and day-to-day living to give older people as much independence, meaning, and quality of life as possible.
How We Work with You:
Whether for a one-time consultation, crisis management, or for ongoing, day-to-day care management, we work with elders, their families, and caregivers to enhance the elders' quality of life, encourage independence, and provide a safe and supportive living environment. Assessments, individualized plans of care and ongoing services are billed at an hourly rate.
