About Us
We work with older adults, families and caregiver in a wide range of situations, helping to navigate the complexities of aging.
We have deep expertise in working with people with dementia, Parkinson’s Disease, neurodegenerative diseases, and other chronic illnesses. We take pride in welcoming clients from the LGBTQ+ community. And we are especially attuned to the needs of people aging without given or chosen family to support them ("solo agers").
Our goal is to enhance quality of life, encourage independence, and provide a safe and supportive living environment. We also offer expert knowledge and support for families during periods of stress and change.
Meet with Your Elder Experts to:
- Have a comprehensive assessment of current needs and discuss resources and next steps
- Coordinate a range of services, including medical, home care, and community supports
- Provide coaching through the challenges of living with chronic illness; i.e., Parkinson's, Dementia, mental health
- Manage medical care as needed including being present at medical appointments and facilitate recommended follow-up care
- Home Safety Assessment with an occupational/physical therapist to address mobility and safety within the home
- Explore appropriate housing options
- Facilitate and promote communication among all involved
- Evaluate current plans to know whether you’re on the right track
- Help during an acute medical crisis
- Help understand the progression of dementia and its challenges
- Support family and those caring for people with mental health challenges
- Understand benefits and insurance eligibility requirements
- Explore and support end-of-life care decisions
- Learn about programs available for caregiver support and respite services
- Explore options to plan for the future
- Assist with moves to Greater Boston as well as moves outside the area
